Week 16:

San Diego has to beat Tennessee
Philly has to beat Denver
Houston has to beat Miami
Indianapolis has to beat New York
New England has to beat Jacksonville
Doesn't matter what happens in Baltimore/Pittsburgh

Week 17:

Houston has to beat New England
Kansas City has to beat Denver

....honestly? I'd put it at about a 20% shot.
